Care Programme Approach

We want to work with you to organise your mental health care. We want to support you and ensure that your needs and choices are central. We know that the system of care can seem very complex.

If you need longer-term care or support from more than one staff member, your care will be organised through the Care Programme Approach. This is the national standard for care delivery. It is the way of assessing, planning, and reviewing your care needs with you.

A staff member will be identified as your care co-ordinator. They will be there to support you. They will also work with you to draw up a care plan that addresses all of your needs. They will then ensure that the care plan is carried out and updated when things change.

If you want more detailed information about the Care Programme Approach, please see our leaflet. It explains how your care will be organized using this. You can also talk to a member of your team who will be happy to answer any questions you have.
