Care Programme Approach

 We want to work with you to organise your mental health care in a way that supports you and ensures that your needs and choices are central in what we know can seem like a very complex system of care.

If you need longer term care or support from more than one staff member your care will be organised through what is referred to as the Care Programme Approach. This is the national standard for care delivery and is the way of assessing, planning and reviewing your care needs with you.

A member of staff will be identified as your care co-ordinator and as well as being there to support you will work with you to draw up a care plan which will address all of your needs. They will then ensure that the care plan is carried out and updated when things change.
