Discharge

Discharge process

When a patient is ready, or needs to be discharged from the ACED service, we will provide a discharge letter with recommendations. This will be sent to the patient, their GP, and any other professionals involved in their care.

Recommendations may include continued physical health monitoring or referrals for specialist investigations, such as DEXA scans.

Consent and confidentiality

We consider a completed referral to indicate the patient’s consent to engage with our service. Any safeguarding or risk concerns will be shared appropriately with relevant professionals involved in the patient’s care.

Mental Health Act and capacity

We follow all relevant legal and clinical frameworks relating to the Mental Health Act and assessments of capacity.

All treatments provided by our service are aligned with evidence-based practice and follow national guidance. In particular, we adhere to the NICE guidelines:
