Specialist Mental Health Practitioners
This is an exciting opportunity for mental health practitioners to work across Primary Care Networks and support GPs across Buckinghamshire.
These roles provide an opportunity for better joint working across primary care, voluntary care and adult secondary care mental health systems. Making it easier for patients to obtain the help they need and sooner.
This is an exciting role for someone who is proactive and passionate about innovation and providing a high-quality holistic approach to patient care for those identified as needing mental health interventions.
You’ll work autonomously as part of an evolving team supporting the PCN in triaging mental health concerns raised by GPs, using your specialist skills and knowledge for upskilling the primary care sector.
You’ll also review patients presenting with mental health concerns, consider a range of treatment options and interventions to provide highly specialised advice and support, and when appropriate signposting to other agencies and liaising with GPs and MH teams.
Experience should include working in mental health services for adults and older age adults with severe / significant mental illness and you’ll also have experience of providing a range of evidence-based interventions to people with a variety of mental health difficulties and serious mental illness.
