How to make a nomination
DAISY describes itself as a partnership with healthcare organisations to provide on-going recognition of the clinical skill and especially the compassion nurses provide to patients and families all year long.
Now any Oxford Health patient, family member or carer on behalf of a patient can nominate a nurse that they feel has gone above and beyond.
All you need to do is fill in the form on this page and your nomination will go to our own DAISY panel for consideration.

Complete this form to share your story of how a nurse made a difference in your care or that of someone you know. We want a detailed account of why this person is so extraordinary and what makes them a stand out nurse for a DAISY award.

What happens next?
Each DAISY Award Honouree will be recognised at a public surprise ceremony in their ward or department.
They will receive:
- A certificate
- A DAISY Award pin
- A hand-carved stone sculpture entitled A Healer’s Touch.
Those who nominate the winners will also be invited although. At present this ceremony will be held virtually.
