Comprehensive, Remote Assessment and Frailty Triage (CRAFT)

Comprehensive, Remote Assessment and Frailty Triage (CRAFT) is a remote consultation approach developed at Yeovil Hospital Frailty Assessment Unit to provide comprehensive geriatric assessment not possible during the COVID-19 pandemic.  It was seen as a way to support primary care colleagues in managing some of their most complex patients, promote collaborative working relationships between primary and secondary care and to provide patient centred care utilising local resources in a timely fashion.
