Dementia Care and Support Planning Generic Toolkit

Following discussions with practitioners, carers and people with dementia, South East Clinical Delivery and Networks (SECDN) has developed the  SECDN Dementia Care & Support Planning Toolkit to support dementia annual reviews and personalised care and support planning. Using this approach will empower and support people to live well with dementia.  The toolkit includes a flow chart guide to the approach and an example of a completed care plan.
