End of Life Care Planning with Young People with Learning Disabilities and their Families

The University of Surrey have worked with Health Education England to develop an educational intervention to assist formal care-givers engage effectively and ethically in end of life care planning with young people with learning disabilities and their families and friends. The Toolkit aims to enable care-givers to develop knowledge and to practise communication skills so they feel more confident and competent to work with young people and their families and friends as they plan end of life care.
The project resources include:
