GP-Consultant Liaison scheme

Since Autumn 2017, over 100 clinicians have taken part, a new GP Consultant Liaison scheme being trialled in Southampton, to break down some of the barriers building up between primary and secondary care.

Secondary care consultants from the local tertiary care provider and local GPs have spent time in each others’ environments to learn about the challenges faced, and their reflections used to put in place action plans to improve delivery of care.
