Person centred care planning – taster session
Thames Valley Strategic Clinical Network are holding free taster sessions across Thames Valley to introduce colleagues to the concept of Person-centred Care Planning and House of Care.
These 2-hour sessions are open to all colleagues working with people with long term conditions and are being held on 3 and 4 December across the region.
