Advance Statements are a way for mental health service users to tell us when they are well what they would want to happen should they get unwell.

The Trust has drafted an information leaflet about this, which includes a proforma for service users. This gives both information about Advance Statements and a template to record one on. Once a statement had been drawn up it could be recorded in the service user’s electronic record.

The leaflet is intended to be widely available in clinical areas as well as the internet site and could act as a focus for care co-ordinators and service users to have a conversation about recording their wishes, choices and preferences.

We are now looking for service users and carers to let us know their views of the leaflet and proforma, so if you would like to be involved please email: enquiries@oxfordhealth.nhs.uk and we will send you the leaflet and information as to how you can respond.
