Community Rehabilitation Patient Flow Lead Team win September’s Exceptional People Team Award

The Community Rehabilitation Patient Flow Lead Team work across all community hospital sites. They are part of the wider Community Rehabilitation Service, recognised by the Community Hospitals Association (CHA) in 2022 for their work in establishing a seven-day service during COVID-19 as part of its Innovation and Best Practice Awards.

The service provides rehabilitation for people who are having increasing difficulty in managing their normal daily activities or who need help to return to a previous level of ability.

This hard-working team continually deliver a high-quality service for the best possible care and health outcomes whilst mirroring the Trust values of providing caring, safe and excellent care.

The team is a collective of brilliant individuals working towards the same goals and vision. They work collaboratively together and with stakeholders, such as Oxford County Council, to ensure the patient and their loved ones are at the centre of their work, thinking of patient needs and helping to make a difference.

The nomination

The team is nominated by the Social Care Team at Oxford County Council. Being nominated by individuals outside of Oxford Health is a real achievement, recognising the impact of the work the team do every day and the importance of the service. Praising the team, the nomination said:

“I wanted to return some kind words about your staff members:

“Since coming to the hospital hub team, Katrina has been amazing. She is so approachable and helpful. She has massively improved communication between health and social care, resulting in good collaborative person centred discharge planning. Even when she stepped away from patient flow, she was always available to take my calls and help direct me to the right person or team, discuss cases or concerns.

“Personally, I am so happy she is returning and I know my team will be too when they hear.

“As well as Katrina, can I also say Tamsin, Michelle, Lucy Knight, Lucy Bint and Jennifer Scarrott have all been brilliant.

“Tamsin, your support and approachability is always appreciated. This job we do can be very tough and stressful at times. All of you and your personalities make each day that much easier. Thank you.”
