Crisis Resolution Home Treatment Team

The crisis resolution home treatment team supports people who have a diagnosis of a mental illness.

We aim to provide care that looks at the whole person. This means we address mental, emotional, and practical needs for a well-rounded recovery.

We offer a range of intense treatments, similar to what you would get in a hospital.

Prioritising mental health

Home treatment is provided at your place, so you don’t need to stay long in hospital. Our experienced clinicians visit you at home.

How it works

  1. Your ward team contacts us about you.
  2. We review your situation and current treatment plans.
  3. We assess you on the ward and identify your needs.
  4. We work with you for up to 6 weeks.
  5. If our service is not suitable for you, we guide you to other helpful services.

  • Wellness Recovery Action Planning (WRAP)
  • Medication ordering, administration and monitoring including depot
  • Carers support
  • Structured daily planning
  • Graded exposure
  • Coping Strategies (DBT) ie 54321, TIPP, Stress Bucket, Distress tolerance, emotional regulation and many more
  • Psychology formulation
