Oxfordshire CAMHS is seeking to improve its communication with you. But before doing so we want to hear what you think about how we communicate now.

It’s important to us that the voices of children, young people and those who care for them are listened to. We can then make the type of changes you want to see.

We’re asking parents and the guardians of children and young people under the age of 18 who live in Oxfordshire and who don’t use our services, to fill in a survey.

We’d like you to be as honest as you can with us when answering.  There are no right or wrong answers and what you say will help us to improve our communication with you and others in future.

What’s more, all entries will enter a prize draw for a £100 voucher. (See terms and conditions below).

The survey is open until March 1, 2025.

 

Terms and Conditions

The competition and prize draw is open to Oxfordshire Residents only; proof of address will be required from any and all winners. 
The prize draw is open to entrants over 12 years of age (staff members are excluded from the draw).
Winners under the age of 18 must be verified by a parent or guardian.
In entering the prize draw, you confirm that you are eligible to do so and eligible to claim any prize you may win.
A maximum of one entry per individual is permitted.
The prize draw will include those who have submitted survey responses up to midnight on the 1st of March. Entries after that time and date will not be included in the draw.
We reserve the right to substitute prizes with another prize of equal or higher value if circumstances beyond our control make it necessary to do so.
The decision of Oxford Health NHS Foundation Trust regarding any aspect of the prize draw is final and binding. Therefore no correspondence will be entered into about it.
The winner will be notified after the 1st of March, via the contact email provided during subscription. We will endeavour to contact the winner by email up to two times – if the winner does not respond to the emails notifying them of their win within 14 days of the second email, they will lose their right to the prize, Oxford Health reserves the right to choose and notify a new winner.
You consent to any personal information you provide in entering the prize draw being used by Oxford Health for the purposes of administering the prize draw, and for those
purposes as defined within our privacy notice.

All personal information shall be used in accordance with Oxford Health’s Privacy Notice.
Oxford Health reserves the right to cancel the prize draw or amend these terms and conditions at any time, without prior notice.

The prize draw and these terms and conditions will be governed by English law. Any disputes will be subject to the exclusive jurisdiction of the courts of England and Wales.
