What do we do

The care home support service team (CHSS), works in partnership with care home staff across Oxfordshire to support the care of residents.

This site will be updated frequently by the clinical team to ensure that the resources provided are up to date and support best practice.

A physical health nurse will visit your care home each month to undertake planned assessments.

  • Assessment /screen of all new residents
  • Falls assessments
  • Supporting residents, families, and carers in decisions about end of life
  • Exclusion of physical health causes for distressed behaviour in dementia and supporting with wellbeing interventions

CHSS referral form

Therapy team
  • Manual handling advice
  • Mobility assessments
  • Postural management advice
  • Seating assessments and advice
  • Any other therapy interventions other than longer term rehab

Mental health team
  • Helping with the management of residents with dementia whose behaviour is challenging.
  • Working primarily with care staff helping them implement psycho-social interventions.
  • The CHSS team will work closely with professionals in other existing health care services especially the community mental health teams.

Each resident’s own GP will always retain medical responsibility.
