Referrals
The Learning Disability and Autism Hospital at Home service is a team for young people who are referred by their consultant psychiatrist. They must meet the following criteria:
- Up to the age of 18
- Diagnosed with a moderate or severe learning disability OR a severe degree of functional impairment associated with an autism diagnosis
- They or their parents agree to treatment
- They are recorded on their local Dynamic Support Register (DSR)
- They are currently rated as RED on the DSR risk matrix
- A Care Education and Treatment Review (CETR) or professionals Multi-Disciplinary Team, (MDT) meeting (including consultant psychiatrist) has been organised or has recently taken place
- Their mental health needs or challenging behaviour is escalating beyond what is possible to continue supporting in the community
- They are at risk of Tier 4 inpatient admission or being detained under the Mental Health Act (MHA)
- (or) they are already a Tier 4 inpatient and require support due to a complex community discharge plan.
Making a referral
If a young person or their carer feels this service is right for them, they should speak to their lead clinician within CAMHS.
To make a formal referral to our team, the young person’s lead clinician within CAMHS needs to submit a referral form to the Thames Valley Provider Collaborative single point of access (SPA) at ThamesValleyT4SPA@oxfordhealth.nhs.uk
Referrals can only be made by a consultant psychiatrist. We encourage pre-referral discussion of individual cases.
