What support & interventions are available from the PCMHT at the Keystone Mental Health & Wellbeing Hub?

Initial contact (triage)

When we have received your referral form, we will contact you.

In the phone call, we will ask you some questions about your mental health and wellbeing, as well as what you would like help with.

We can give you some information about the Hub, and we can decide together what the best plan is for you.

Assessment

The assessment will be face to face or on Microsoft Teams, depending on your preference. You can bring a friend or family member for support if you like.

In the assessment, you will be asked about different areas of your health and wellbeing. You will be asked about whether you have concerns in these different areas, and what help you would like. At the end of the assessment, you will be supported to develop a care plan.

PDI workers

Structured Psychological Support: This may be offered to you if you experience emotional difficulties that affect the way that you feel about yourself, or your relationships with others.

It involves up to ten individual sessions, either face-to-face, over the phone or online. SPS can help you to understand how your past experiences influence how you think, feel and act.

You will be asked about the biggest problems you are having, and we will agree on one or two of them to focus on. In the rest of the sessions you will be given skills to tackle these problems.

Brief intervention

For support in managing crisis and collaborating on creating a safety plan together with you and your trusted friends, family or other support networks.

IPS

Long-term mental health conditions can make it difficult to find a job or to stay in work, and you may need support to get back into work.

We work with the Individual Placement and Support (IPS) team and have an Employment specialist in Keystone Abingdon who will help you reach your employment goals.

The specialist will build a picture of your employment needs and goals, before working with you to agree a plan moving forward.

They can also help if you want to retain your current job and are having difficulty doing that.

Peer Support

We provide non-directive support, via a combination of group and one–to one work, to aid you in identifying your own strengths, hopes, goals and needs.

We can support you to independently explore education, leisure, social, employment and volunteering opportunities.

We aim to bring about cultural and structural change by introducing lived experience as a form of expertise within mental health services.

We can support you to identify and gain the skills you may need in order to achieve your wellbeing and mental health goals.

We can help enable you to access other recovery-based services including courses offered by The Recovery College, Mind, and Restore
