Community Therapy Service – Information for patients

What is the Community Therapy Service (CTS)?
We provide short-term rehabilitation seven days a week 8.00am to 8.00pm to people in their own home who:
- Have had a recent illness, injury, or deterioration in physical and/or mental health.Are finding it recently more difficult to carry out daily activities.
- Need help to return to a previous level of ability
e.g. after hospital admission.
Aims of the service
- Help you achieve your short-term rehabilitation goals.
- To increase or maintain a person’s level of independence in aspects of daily living at home.
- Prevent admission or support discharge from hospital or care home.
- Improve quality of life
- Provide advice on self-management.
What to expect
- Assessment of needs by telephone, video consultation or face-to-face visits as required.A time limited short course of treatment to reach certain agreed goals.
- Provision of basic equipment, aids, exercise programmes or other relevant treatment
- Onward referrals and signposting to other services
Meet our team
- Occupational Therapists who will assist in regaining independence in home and local environment.
Physiotherapists to help recover movement, strength, and mobility. - Mental Health Practitioners who can support conditions e.g. anxiety, depression, or memory problems.
Assistant Practitioners who help clinicians above with assessment and treatment. - Rehabilitation Assistants who support patients to achieve their goals as set by the above clinicians/practitioners.
Administrators, Students and Apprentices.
Making a referral
The service is available for anyone 18 years or older who is registered with an Oxfordshire GP. Referrals can be made by any healthcare professional, the person themselves, their family member or carer by contacting us.

Patient Participation Group (PPG)
This is a group of volunteer patients, relatives and carers that meet regularly with our CTS health care professionals to discuss, support, and improve our services.
If you are interested in joining, please ask your CTS Professional.
No training is required to become a member.
We would like to listen to our patients’ perspectives on services that are provided and contribute to continuous improvement.
Patients will have a forum to suggest positive ideas and voice concerns.
