Neuro-Inclusion Passport

The Neuro-inclusion Passport helps neurodivergent people share their needs with their primary care team and other professionals.

It also supports making reasonable adjustments in healthcare and community settings.

How it helps

Completing this can:

  • Help individuals understand themselves better.
  • Help others around them to better understand.
  • Identify adjustments that are needed for individuals to access services.
  • Help communicate to professionals reasonable adjustments that are needed and advise on implementing them.

How to complete it

You have the option to complete the passport yourself. Alternatively, our staff can assist you with it when you attend your appointment.

What happens next?

After completing the passport, please print it out and bring it to your appointment.

Hand it to your clinician, and we will add it to your patient records, making it accessible to any of our staff who work with you.
