We again offer our sincere condolences to Mr Austin’s family. We acknowledge the findings of the independent report into the care and treatment of Mr R.

While the independent investigation report concludes that the clinical team caring for Mr R could not have predicted or prevented the violent incident in 2013, the Trust recognises that there is learning from this incident. We have taken account of the recommendations of our own investigation in 2013 and the independent investigation and we have completed every action in relation to the recommendations made.

We are committed to learning from incidents and tragic deaths like this and we continue to work to improve and develop our services.
