Hospital at Home — Oxfordshire

Patients on Hospital at Home stay in their own homes but they recieve extra care and attention from the Hospital at Home Team. The team work like a hospital ward team and have regular multi-disciplinary meetings where they discuss the patients they are looking after. The service is designed to give patients extra support so that they are not admitted to hospital or so that their admission is as short as possible.


Community Matron Service — Oxfordshire

The Community Matron Service offers support to patients with Life limiting illness.


Community Type 2 Diabetes Service — Oxfordshire

This countywide service is staffed by five Diabetes Specialist Nurses (DSNs) who work with GP practices, community hospital staff and other community health care teams to help them support adults with Type 1 and Type 2 diabetes.


Minor Injuries Units and First Aid Units

Minor Injuries Units are for injuries, such as deep cuts, eye injuries, broken bones, severe sprains, minor head injury, minor burns and scalds. Many people go to Accident and Emergency departments when they could be treated just as well and probably quicker at a Minor Injuries Unit.

Falls Prevention Service — Oxfordshire

Falls are not just the result of getting older and many falls can be prevented. The Oxfordshire Falls Prevention Service offers support and advice to help people avoid falls and regain confidence if they have experienced a fall.


Oxygen at Home Assessment Service — Oxfordshire

The Home Oxygen Assessment service assesses, prescribes, reviews and follows up patients in Oxfordshire requiring oxygen at home.


Integrated Community Respiratory Services

The Respiratory Care Team offers a range of support services to enable patients to be cared for at home where possible, rather than be admitted to hospital. The team helps over 200 patients every month cope with chronic respiratory conditions such as asthma, emphysema and chronic bronchitis.
