Referrals

To access our service, patients need a referral from a GP or another healthcare practitioner, such as a health visitor. Healthcare practitioners can refer patients to us via Healthshare.

All referrals must include a fully completed referral form. Where appropriate, this should also include supporting documents such as fluid balance charts and output diaries.
