About us

The Children’s Community Nursing Team provide vital support to children and young people from birth – 18 years of age with complex and nursing needs across Oxfordshire. The child is required to have an Oxfordshire GP.

The service encompasses several service provisions which we are commissioned to provide within each of our four localities (Central, South, North and West) including the following:

Acute services

Providing nursing care at home for children requiring short term acute care such as wound care, medication administration such as intravenous antibiotics and subcutaneous medications.

Hospital at home

Providing nursing support for children and young people who require ongoing support following an acute illness and continue to require further assessment following admission to hospital or visit to their GP.

Providing support for children with complex health needs requiring intervention such as enteral feeding, respiratory support such as oxygen therapy or support using equipment such as a ventilator.

Respite care

Providing respite care for families on the CCN Caseload as well as supporting care packages for children with continuing health care needs.

Providing education to Staff and training for External Care providers on Delegated HealthCare Tasks.

Offering holistic care to children and young people and their families, when they are on a palliative care pathway.
