Children’s community nursing

The children’s community nursing team provides support to children and families with complex and nursing needs by empowering the family to care for their child or young person at home.

In order to do this, we work with professionals from health, education, social services and voluntary sectors to ensure the child or young person is cared for at home and is able to attend education safely.

We work closely with our discharge co-ordinator team to support and facilitate early discharge from hospital and prevent readmission where appropriate and safe.
