Children’s community nursing

What we do

The children’s community nursing team supports children and families with complex nursing needs.

We empower families to care for their child or young person at home.

Who we work with

We work with professionals in health, education, social services, and the voluntary sector.

Together, we make sure each child or young person can be safely cared for at home and can attend education.

Discharge support

We also work closely with our discharge co-ordinator team.

This helps children leave hospital early when it is safe and prevents unnecessary readmissions.
